Medication Administration
Wellspring can provide Medication Administration! Below are the beginning steps to get started.
*VERY IMPORTANT*: Do not send any medications to Wellspring Community without contacting Morgan Minardi at least 1 week prior to when the medication will need to be administered. We cannot administer medications without prior approvals and authorization paperwork completed in advance. Thank you!
- Please review our medication policies which are linked to the right.
- Here is Wellspring’s Mediation Authorization form. This form is required to be completed and signed by the participant’s doctor for the prescriptions that will be administered at Wellspring. Please print out this form.
- Have your doctor complete pages 1-2, and sign page 4
- Participant and/or legal guardian will complete page 3 and sign page 4
- Be sure each medication section is reflective of EXACTLY how the medication will need to be administered. Wellspring is unable to administer any medications without a clear and specific order from the prescribing doctor.
- When this form has been completed with ALL medications that will be administered at Wellspring, please send, or return to Morgan Minardi. This can be done through email, regular mail, or in person. Wellspring cannot accept a picture of the form.
- For medications that required a Human Rights Committee review, please contact Morgan for additional paperwork. (303) 660-1935
