Overview

Wellspring Community is a provider agency enabling us to bill Medicaid waivers for many of the services we provide. More than 95% of the individuals enrolled at Wellspring rely on Medicaid waiver funding to access these services. As a provider agency, we are authorized to bill under both the Supported Living Services (SLS) and Developmental Disabilities (DD) Medicaid waivers.

Services Offered Day Program (DD/SLS)

Specialized Habilitation (SH)

Wellspring offers community-integrated, non-residential services that promote self-sufficiency and independence through classes and cooperative opportunities focused on daily living skills, personal growth, and reinforcing existing skills and abilities.

Supported Community Connections (SCC)

Wellspring offers community-focused services that help individuals build skills and relationships to help them engage in everyday community life through classes that provide volunteer opportunities, outings to local sites, and partnerships with other agencies like the rec center, library, and more.

Non-Medical Transportation (NMT)

Wellspring provides transportation services to enrolled individuals along 5 designated bus routes, so they can access day habilitation services and work opportunities.

Services Offered Residential (SLS)

Individual Residential Services and Supports (IRSS) - DD Wavier only

Wellspring provides individualized services and support to residents of Unity on Park that focus on skill-building and daily living to promote their habilitation and greater independence.

Mentorship SLS

Wellspring provides services that promote self-advocacy for residents at Unity on Park by providing instruction, guidance, and meaningful experiences at weekly community social events.

Personal Care SLS

Wellspring provides services to Unity on Park residents that support an individual's physical, maintenance and supportive needs through supervision and/or cueing.

Homemaker SLS

Wellspring provides services and support to residents at Unity on Park that help an individual maintain a healthy and safe home environment and promote their independence in cleaning, laundry, and household care.

How can I apply for Medicaid Waiver Funding?

Contact a Case Management Agency (CMA) in your area. A few CMAs that support individuals in our program include Developmental Pathways, The Resource Exchange, Rocky Mountain Human Services, and Developmental Disabilities Resource Center.

Yes, a person can use SpecHab units for online program. However, SCC cannot be utilized for online programming.

No, Wellspring does not provide Supported Employment or Prevocational services. The Cooperative Opportunities are taught by Wellspring instructors and are billed as SpecHab.
